MyChart Terms and Conditions

UVA MyChart is a personalized, secure way to access portions of your medical information and to communicate with your physician online. With UVA MyChart, you can:

  • View select test results
  • Communicate about medical appointments
  • Request a prescription renewal
  • Provide information about your health history and your family health history
  • Request non-emergency medical advice

UVA MyChart is not your official UVa medical record, and may not contain all the information that is in that record. You can get a copy of your official medical record from Health Information Services.

We think it is important for you to know how we handle information we communicate via the Internet. This Terms & Conditions statement outlines our practices and our sensitivity to your right to privacy. We reserve the right to revoke access at any time for any reason.

You must be 18 years of age or older to request activation of a UVa MyChart account.

Electronic Communication

University of Virginia Health System will make its best effort to provide you a timely response to your electronic messages. In some cases, the physician or clinic staff member may not be available immediately, so you should allow at least three (3) days for a response. Emergency matters requiring immediate response cannot be submitted electronically. Also, there may be times when UVA MyChart is not available.

We can only respond to electronic messages based on the information you provide. If we do not have enough information, we will not be able to provide accurate and reliable services or information.

You will be notified via e-mail when there is new information that you can view in UVA MyChart. This means that anyone with access to your e-mail will be able to see this notice. Although no private medical information will be sent in the e-mail notice, you may wish to consider this when you provide an e-mail address. If you wish to receive e-mail notices, it is your responsibility to keep your email address up-to-date in your MyChart profile.

Proxy Access For Children Under 18 Years of Age

If you choose to give proxy access or wish proxy access for your child (under 18 years old) these same Terms and Conditions apply when you access the MyChart account of another patient.

Security and Confidentiality

We afford the same degree of confidentiality to medical information stored in UVa MyChart as is given to any medical information stored by UVa Health System. We are committed to protecting the confidentiality of your medical information. We limit UVa Health System employees’ access and ability to enter or view information based upon their job duties. Firewalls, passwords, encryption, and audit trails are used to safeguard your information. We have taken steps to keep all information received electronically as secure as possible against unauthorized access and use.

Your user name and password provide two layers of authentication and are stored in an encrypted database that is isolated from the Internet. As a UVA MyChart user, your role in maintaining the security of your medical information is to:

  1. Change your password on a regular basis
  2. Keep your login ID and password confidential.

The security of your login and password is your responsibility. If you think your password has been compromised, please change it immediately.

Appropriate Use

Access to UVA MyChart is granted to give you information about your medical care. Improper use of this system may result in your MyChart access being terminated at the discretion of the UVa Health System.

Termination or Deactivation of UVA MyChart Access

UVa Health System reserves the right to terminate access to UVA MyChart at any time and for any reason. The following is a list of reasons for termination of UVA MyChart access. However, this list is not all-inclusive, and each circumstance will be evaluated individually:

  • Sharing of your login and password with another party.
  • Inappropriate use of electronic messaging (excessive use, inappropriate or abusive content)
  • You are no longer a patient of the practice/physician
  • Pending legal issues with the physician or UVa Health System
  • Inappropriate use of UVA MyChart customer service
  • Other activities that cause a disruption to operations
  • Patient is incarcerated or in custody of law enforcement

Automatic deactivation will occur if the number of failed login attempts exceeds established limits.
